About 60 minutes by video. Your history, current symptoms, what you have already tried, medical background, and what matters to you.
You leave with a documented direction: what we believe is happening, what we recommend, and what we expect to change.
Around 30 minutes, every few weeks at first. This part is not optional — medication needs monitoring for safety and for effect.

David has spent 22 years in behavioral health. Seven years as a certified nursing assistant on a behavioral health unit, then fifteen as a registered nurse — through psychiatric hospitals, crisis care, detoxification, admissions, and behavioral health leadership. He now holds a Master’s in Nursing as a Psychiatric Mental Health Nurse Practitioner.
He built Firefly Global LLC as a solo practice on purpose. What he kept seeing, over two decades, was not only a shortage of clinicians but a system that burns out the ones it has — administration, documentation, and broken handoffs taking time away from patients. This practice is deliberately small so that does not happen.
You should never have to guess what is happening with your own care. You will hear the clinical reasoning, the alternatives, and how progress will be measured.
Your history is held by one person, not reconstructed at each visit.
Adjustments come with a rationale. Side effects are treated as seriously as symptom improvement.

Fees and payment

The practice currently operates on a cash-pay basis. Payment is due at the time of service.
Insurance access is being added. We are not putting a date on it until the contracts are signed. Leave a contact and we will get in touch the day it opens.

Before your first appointment you receive a written Good Faith Estimate, as required by federal law for self-pay patients. Nothing is charged that was not estimated.
Every visit comes with an itemised receipt you can submit to your insurer. Please give at least 24 hours’ notice to change or cancel an appointment.

Appointments run into the evening and through the weekend, so care fits around work and school.

Questions
The first appointment is $150 for 60 minutes — a time-limited launch price, normally $250. Follow-ups are $120 for 30 minutes, normally $150. You are given a written Good Faith Estimate before your first appointment, so the figure is confirmed before anything is charged.
No. You can book directly. If your therapist or doctor has suggested a psychiatric opinion, that is useful context to bring, but nothing needs to be arranged beforehand.
Appointments are available Monday to Friday in the evening and throughout the weekend. Submit a time and we confirm it, usually the same day.
Often, yes — when the picture is clear enough to prescribe safely. Where more information is needed, or where something needs checking first, a prescription may wait until the next appointment. Either way you will be told why.
Yes. All appointments are by secure video, anywhere in California. There is no clinic to travel to.
Not yet. The practice is currently self-pay. Insurance contracts are being worked on and we are not putting a date on it until they are signed. Leave your details and we will tell you the day it opens.
Yes. Care for adolescents includes appropriate coordination with parents or guardians, while keeping clinical confidentiality where it applies.
These are prescribed only in limited circumstances, with careful documentation and monitoring. That is discussed openly at your evaluation so you know what to expect.
Both. Medication management is the core, and talking approaches including cognitive behavioural therapy (CBT) can be part of your care.

Firefly Global LLC is an outpatient practice and is not a crisis service. We do not provide emergency or crisis stabilisation, and messages are not monitored around the clock.
