Privacy Policy

Last updated 18 September 2026. This policy covers this website and the information you send through it. It applies to Firefly Global LLC, a psychiatric practice operated by David Fairbanks, PMHNP-BC (Psychiatric Mental Health Nurse Practitioner, Board Certified), seeing patients by video across California.

This policy, or your medical record?

There are two separate documents, and they cover different things. This page covers what this website collects when you visit it or contact the practice through it. The Notice of Privacy Practices covers your medical information once you are a patient — how it is used, who may see it, and the rights you have over it. If you are looking for the second one, that is the page you want.

What this website collects

You do not need an account to read this site, and nothing here asks for your medical history. The site collects only:

• What you choose to send. If you use the insurance enquiry field, the email address or phone number you enter.
• Standard technical information that any web server records in order to serve pages and keep the site working, including your Internet Protocol (IP) address, the pages you asked for, and the time. These are ordinary server logs.
• How you arrived. If the address you clicked included tracking parameters — the campaign tags commonly called UTM (Urchin Tracking Module) parameters — the practice records which source, medium and campaign they identified, together with the referring page. This is about the link, not about you.

Booking an appointment

Appointments are booked through the Tebra scheduling widget embedded in the practice page. That widget is operated by Tebra, not by this practice, and information you enter into it is handled under Tebra's own privacy terms. Once a booking request reaches the practice it becomes part of your clinical record and the Notice of Privacy Practices applies.

Cookies

This website does not set advertising cookies and does not run third-party advertising trackers. Any cookies that are set come from the practice's own hosting or from the embedded scheduling widget, and are used to make those functions work.

What is never done with your information

Information collected through this website is not sold, and is not used for marketing unrelated to your own care. It is not shared with third parties except the service providers needed to run the practice — the electronic medical record and scheduling platform, and the hosting provider — or where the law requires it.

How it is kept

Transmission to and from this site is encrypted using HTTPS. Clinical information is held in the practice's electronic medical record system, with access limited to those who need it to provide or bill for care.

Children

Adolescents are seen by the practice with appropriate involvement of a parent or guardian. This website is not directed at children, and nothing on it should be used to send clinical information. Please contact the practice by phone instead.

Your choices

You can ask what information the practice holds about you, ask for it to be corrected, or ask the practice to delete information it is not required to keep. There is more detail, and the formal rights that apply to medical records, in the Notice of Privacy Practices.

Changes to this policy

This policy may be updated. The date at the top of the page shows when it last changed, and the current version always applies.

Getting in touch

For any question about this policy, or about information held about you, call or text 909-203-3184. A formal privacy complaint can also be made to the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalised for complaining.
