Notice of Privacy Practices
Who this notice covers
This notice describes how medical information about you may be used and disclosed, and how you can get access to it. It applies to the practice and to anyone working on its behalf.
What we collect
Information you give us directly (your name, contact details, history, symptoms, medications, insurance details if relevant), information created during your care (assessment notes, diagnoses, treatment plans, prescriptions), and billing records.
How we use it
To provide your care and coordinate it with other providers you tell us about; to prescribe and to check for interactions; to bill you and, when insurance applies, to bill your plan; to run the practice, including scheduling and reminders; and where the law requires it.
What we do not do without your written permission
We do not disclose information for marketing, and we do not sell it. Uses and disclosures not described here require your written authorisation, which you may revoke at any time.
When the law requires or permits disclosure
Examples include serious and imminent risk of harm to you or another identifiable person, suspected abuse or neglect of a child or a dependent adult, court orders, and public health reporting. When we can, we tell you before doing so.
Your rights
You may ask to see and get a copy of your record; ask us to correct information you believe is wrong; ask for limits on what we share; ask for a list of certain disclosures; ask us to contact you in a particular way or at a particular place; and get a paper copy of this notice. We will tell you if a request cannot be granted, and why.
Keeping it secure
Records are held in the practice's electronic medical record system. Access is limited to those who need it to provide or bill for your care. We review how records are handled and will notify you if a breach affecting your information occurs.

If you believe your privacy rights have been violated you may complain to the practice or to the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalised for complaining.
Changes to this notice
We may change this notice, and the new version will apply to all records we hold.
